Handoff & Context Distillation Plugin (migrated from Pi ecosystem handoff.ts).

Provides the handoff tool. It stores a child conversation that starts with the summary and goal that the model supplies, and then ends the current turn. It does not run the child conversation, and it does not summarize the parent itself.
